Choice Healthcare Durable Medical Equipment

Durable medical equipment · Arizona & Nevada

Most equipment delays aren't supply delays. They're paperwork delays.

A power wheelchair doesn't sit in a warehouse waiting on a truck — it sits in prior authorization waiting on a chart note nobody wrote. Choice Healthcare sends a representative to the point of care and builds the documentation packet on the spot.

As fast as 3–5 weeks from referral to delivered equipment when the documentation is complete.

How it works

Three steps, and we own two of them.

You send a name

A call, a fax, or the form below. No packet, no cover sheet — the patient's name, a callback number and the ordering practitioner is enough to start.

We build the file

A representative meets the patient at bedside, in clinic or at home, confirms the functional limitations with the treating practitioner, assesses the residence, and assembles the documentation packet against the coverage criteria for the specific codes being requested. What that involves →

We deliver and follow it

We submit the prior authorization and handle any denial or appeal ourselves rather than returning it to you. Equipment is set up and fitted in the home, and rented items are maintained at no charge.

For referring providers

We take the documentation burden, not just the order.

You identify the need. The work that actually decides whether the equipment gets approved — chasing signatures, dates and necessity language — lands on your desk by default. It doesn't have to.

For patients & caregivers

Someone comes to you. Nothing gets mailed back and forth.

You should not have to project-manage your own wheelchair. If your doctor has said you need equipment — or you think you might — call us and we'll take it from there.

What to expect →

How to refer

Three ways in. All of them end with us doing the paperwork.

Fastest

Call

Reaches a person now. For an urgent discharge this is the only route that should be used.

For providers

Fax the referral form

One page. Print it, fill in what you know, fax it. Fax is a covered channel for patient information; email is not.

For patients

Take the checklist to your doctor

The one-page list of exactly what your physician needs to write in the note. Two minutes of their time can save two months of yours.
